LWD Authorized
    Collection Centers

(for Members & Non Members)
LWD has a large network of DOT Authorized Collection Centers for you to choose from. Each center is equipped to handle drug and alcohol collections and will process your proper CCF or Alcohol Testing form with our designated DOT Approved Laboratories for processing.

If you cannot send your driver to LWD's primary collection facility or arrange for an onsite visit, choose from one of the many sites by using the search utility below.
You must obtain a Custody and Control Form (CCF) from LWD PRIOR to arriving to the collection center.		  
 
Please follow these steps:

  
    1. Obtain a Custody and Control Form from LWD
    PRIOR to arriving at the collection center, contact LWD at (626) 813-1086 or request one online. LWD will mail the authorized form to you at no charge.
    2. Search for an LWD/Quest Approved Collection Center
    When you click on the Search for Authorized Locations button, you will see the Collection Site Locator screen below. Then:

        - Enter your Zip code
        - Enter Radius in miles from your location
        - Check mark Drug / alcohol screening
        - Check mark for a regulated employer
        - Check mark Urine drug screen
        - Click Search

    3.

    After you have searched and chosen the collection site of your choice, contact them directly to schedule an appointment for your test. Most collection sites offer walk-in service without appointments. Remember to take your LWD CCF form to the site with you.
